Ethical Challenges Surge as GLP-1 Weight Loss Medications Thrive

The increasing popularity of GLP-1 medications for weight management raises significant ethical concerns, particularly regarding the treatment of patients who may be losing excessive weight. Dr. Jody Dushay, an assistant professor of medicine at Harvard Medical School and an attending endocrinologist at Beth Israel Deaconess Medical Center, has expressed her struggles with the implications of prescribing these drugs, especially for individuals who do not have type 2 diabetes.

In a recent article, Dr. Dushay recounts the challenges faced in managing patients who seek to achieve an unhealthy weight. Since the introduction of GLP-1 therapies, which are designed to aid in weight loss, the medical landscape has experienced rapid shifts. In 2023, she highlighted issues of medication shortages that led to difficult decisions on who should receive treatment. Today, her focus has shifted to the ethical dilemmas posed by patients who fixate on weight loss, often overlooking their overall health improvements.

Dr. Dushay categorizes her patients based on their responses to GLP-1 treatments. The majority lose between 10% and 20% of their body weight within six to twelve months, maintaining much of that loss while continuing the medication. A small percentage, around 5%, do not respond or tolerate the therapy, while another 5% are considered super responders, achieving weight loss exceeding 25%, with some losing as much as 45%
