Identifying systemic mastocytosis early can significantly impact patient outcomes, as highlighted in a recent session at the 2025 Society of Dermatology Physician Associates (SDPA) Fall Conference. The session, titled “When a Rash Is More than Skin Deep: Systemic Mastocytosis Essentials,” featured insights from Hussein Abbas, MD, PhD, an assistant professor at the University of Texas MD Anderson Cancer Center in Houston.

Understanding Systemic Mastocytosis

Systemic mastocytosis is a rare condition often first identified through skin manifestations. In an interview with HCPLive, Dr. Abbas emphasized the challenges both physicians and patients face due to the rarity of the disease. “Systemic mastocytosis is a very rare disease, but it’s also a significant disease because it can create a lot of symptom burden to patients and can impact longevity and quality of life,” he stated.

During his presentation, Dr. Abbas aimed to equip healthcare professionals with diagnostic pathways, key clinical clues, and collaboration strategies with hematology and oncology specialists. He underscored the importance of recognizing systemic mastocytosis, particularly by dermatologists and allergists, who often see the initial skin expressions of the disease.

Clinical Clues and Management Strategies

Dr. Abbas pointed out that many patients with cutaneous mastocytosis also have systemic involvement. He categorized the cutaneous manifestations into three main groups, highlighting maculopapular cutaneous mastocytosis (MPCM) as a common form. “MPCM is characterized by brown, reddish lesions that can be palpated. They typically appear uniform in shape and can elicit a reaction when stimulated,” he explained.

For effective diagnosis and management, Dr. Abbas recommended blood tests to measure specific enzymes, such as tryptase. He noted, “Recognizing this condition requires a thorough workup, which is essential for proper treatment.” His session aimed to foster better understanding and collaboration among healthcare providers to improve patient outcomes.
